Bent ammo cases from AK

Over the last couple times I've gone shooting, my AK has sorta jammed and the bullet is sitting at like a 90 degree angel in the chamber with a big ding/cut on the outside of the shell casing. I've noticed that all my empty casings when I pick them up off the ground have a dent in them as well in the same area. The dent is just below where the case becomes "full size" right below the indention before where the bullet itself is.

Any ideas on what's causing this?

my first guess would be a magazine issue, it sounds like your mag is tilted forward or is riding too high.

I'd try a different magazine, carefully install it, making sure it sits properly in its groove on the front side.

If that doesnt fix it, point the gun down range, and cycle the bolt manually while watching where it loads the shell. You may be able to see the issue.
